With some people, this is due to a chronic sinus infection. You can test
for that by trying an over-the-counter sinus medication and seeing if that
eliminates the breath problem.
If sinus infection is the cause, what you do next is trickier. Often, the
root cause is a food allergy. In my case, it’s any kind of hot peppers. Finding
which food it was the cause was a matter of luck.
Garlic supplements help.
You might have a digestive problem. If you have continuous bad
breath. You should see your doctor.

I had the same problem for years and tried a number of avenues such as watching what I eat and brushing my entire mouth and a wide assortment of mouthwashes. I relocated and was very careful about the dentist I chose. I was glad I was. He said my gums had problems due to calcification just below the gum line. I went through the mildly painful experience of having the calcification removed. Then on his advice, I made it a point to floss every day, properly and without fail. In a matter of weeks, there was a reduction in bleeding when I brushed and my breath was much cleaner.
I was disappointed in the fact that the previous dentists focused entirely too much on the teeth and I had been neglecting my gums out of ignorance. That was two years ago and my gums have not completely recovered. However, my wife rarely complains about my breath any more and my gums rarely bleed. The majority of the cure was a few rolls of dental floss and a little education.
